Best Hair transplant Centre In Vizag, Visakhapatnam, Andhra Pradesh, India

Dr. VJs Cosmetic Surgery & Hair Transplantation Centre Vizag: Hair transplant has been around for a decade or so, but the mechanism has improved considerably over the years, with the introduction of new techniques. The present scenario makes the process of hair restoration a unique venture. With the evolution of FUE, bio-FUE and many more, the process has shown great diversification which yields only perfect results .While FUT hair transplant in India has been available since a long time ,FUE and Bio-FUE are comparatively new techniques.

Hair Transplantation Technique

  • Although no age is considered unsuitable for hair transplant, it is usually not preferred in the age group of 20-30 years. In case the patient of this age group shows a stabilized rate of hair loss, then they can be considered for the surgery.
  • An overview of family history is also taken into account. The next step is to assess the quality,  color, and density of the hair.
  • The contrast between the hair and skin color is also taken into account. The results are better if the contrast between the hair and the skin is less.Hair Transplantation Technique

Different Types Of Hair Transplants

Hair transplant is of two types-

FUT hair Transplantation

Follicular Unit Transplantation (Fut)

FUT starts with the numbing of the scalp and removal of a skin strip from the donor area. This strip is dissected with the special microscopes. The division results in thousands of grafts, which contain an individual follicular unit. After stitching the donor area, the surgeon creates tiny holes on the recipient site. The surgeon then places one follicular unit each into these holes. A single FUT session might last for 5 – 8 hours. The time taken depends on the number of grafts.

FUE hair Transplantation

FUE &Robotic FUE

It involves FUE process and if assisted by a robot,it is called Robotic FUE. In FUE, the follicular units are extracted using a small cutting instrument. The grafts are then implanted on the recipient site, one by one. The holes heal up in about a week’s time, leaving a tiny scar. The introduction of robotics has improved the procedure in many aspects, but most efficiency has been achieved in terms of eliminating human error.

Grafts

Grafting includes removal and insertion of a portion of the scalp. After removal, the hair strands are inserted into a section of the scalp. The strands are kept healthy by administering saline injections periodically. The portion with the healthy hair is reattached using surgery after the transplant has been done. It may require multiple sessions. The scalp is left to heal after this.

Flaps

This is an improved way of conducting a hair transplant. They score over grafts with the fact that they provide wider and faster coverage of the scalp area. In this procedure, the bald area is removed surgically. After that, healthy hair is stretched and tethered over the area. Although it results in more scarring ,it provides more coverage of the scalp. This also means that the scar will most probably be covered by the healthy hair and you will not have to worry about it showing.

Tissue Expansion

This procedure involves placing a tissue expander under the scalp with healthy hair growth. After a few weeks, the tissue expander enables stretching and expansion of the tissues as it gets filled with salt water. It means expansion of hair-bearing tissue which can be grafted after sufficient stretching.

Revision Hair Transplant

Revision hair transplant refers to the rectification of the past hair transplants. If you had the surgery done 20 -30 years ago, chances are that you have the irregular, pluggy look which might be a constant source of embarrassment for you. In that case, you can get the errors rectified, this procedure is called Revision Hair Transplant. It will also work for people who are left with an ugly visible scar which has not been covered up by the hair growth. It can also be used for irregular hair lines and unequal distribution of hair density.

Preoperative Preparation

  • The patient is made to shampoo his head with Betadine surgical scrub on the day prior to and on the day of the surgery.
  • Preparation of the donor area

The donor area is prepared for the transplant by trimming the hair to a length of 2-4 mm. A local anesthetic solution, made up of 30 ml  2% lignocaine and 100 mL of normal saline, to which 1 mL of adrenaline (1:1000) has been added. Normal saline solution is also injected into the donor area. After 10-20 minutes, the donor area is ready for the surgery, with chances of minimum pain and bleeding.

Harvesting

The donor strip is harvested using a single bladed or multi-bladed knife. It is better to use a single bladed knife as the incisions by multi-bladed knife might cause damages to the follicles. The harvesting process has to be carried out with utmost care. It should be ensured that the blades remain parallel to the direction of the hair. This is necessary to ensure that the hair roots don’t get damaged. The hair is harvested and after that, the gap is covered using staples or sutures. The surgeons are careful to take the cuts to the dermis so as to avoid damage to the deeper roots of the hair.FUE hair transplant in Indiaisa readily available option.

FUE involves the removal of a follicular unit from a donor area using 1 mm punch. Although it is touted to be a scarless method of hair grafting, it does leave multiple scars which are in the form of dots. These may be larger than the scars left by other methods.

The harvest of light colored hair is more difficult than dark hair. For ensuring the proper visibility, the patients with light colored hair are requested to dye them. The surgeon can also inject  Methylene blue in a very low concentration into the donor area to effectively recognize grey hair. The harvesting process needs to be done really carefully in people who are having the procedure done the second time. This is due to the fact that the

scars from the previous surgery might have an adverse effect on the hair in the donor area.

strip harvesting

Graft Preparation

As soon as the donor strips are extracted, they are immersed in chilled saline solution. To achieve this aim, the tray containing the grafts is kept immersed in saline on ice. This is a crucial step for successful surgery as it ensures proper hydration of the grafts, which increases the survival chances of the grafts and the success rate of the surgery. In case a single strip has been harvested, it should be divided into small pieces before the individual grafts can be extracted out of it. Good illumination is another requirement which is absolutely essential during graft cutting. Wooden tongue depressors or a vinyl dissecting surface with a backlighting system must be used for this purpose. A dissecting stereo microscope should be used for the dissection. Although it slows down the process, it lends accuracy to the whole process.

Preparation Of The Recipient Area

The recipient area should also be under the influence of anesthesia. Usually, a supratrochlear and supraorbital nerve block is used for the purpose. This area should be tumesced with saline solution. Adrenaline is preferred in many cases but the use is restricted to men for medical reasons.

We have already mentioned that the surgeon needs to be careful while making the clots in the head. While scalpel blades are not the ideal instruments for making holes, other instruments are not easily available in India. The scalpel is unsuitable because it often cuts deeply resulting in floating of the inserted grafts. They also cause significant damage to the hair in case the patient is a woman. To eliminate such complications, an instrument called ‘Kolkata Slit ‘ has been recommended.  It is an instrument which can be reused and available in different sizes. The instrument is effective in ensuring that the gap created is just according to the size of the graft and the direction of orientation is maintained. The instrument is also effective in increasing the density of areas with scanty hair growth.

Graft Insertion

In this step, the grafts are placed into the recipient slits. The instrument used for this purpose is fine angled forceps. Minimum damage is desired, therefore the grafts are extracted in such a manner that about 2mm of subcutaneous fat is extracted. Using steady pressure the grafts are positioned on the recipient site. The surgeon must avoid burying the grafts below the skin level as it will lead to an unnatural appearance and lead to the formation of cysts. A team of two or three persons can be used to insert the grafts to endure efficiency and speed. A typical session lasts up to 5-6 hours which involves the grafting of 200-30 grafts.

Postoperative care

Usually, the patient can be discharged the same day. Most of the surgeons don’t prefer bandaging the area but some may do it. If the area is bandaged, then it must be done carefully. Removing the bandage too will require a lot of care as the grafts might stick to the surface of the bandage.

Redness and swelling to some extent are expected and the patient should be prepared for it. Surgeons might use oral steroids for 3-5 days to reduce edema. Some medicines might be injected to reduce the swelling. The patients are usually advised to wear a headband on the face to avoid the swelling from coming down to the level of the face. The patient can wash his hair with a mild shampoo on the second or third day. The patient must be careful not to strike the tooth of the comb against the transplanted hair while combing.

The patient must avoid wearing tight t-shirts which need to be taken off over the head. The patient is also advised to not to use hair oils and strong shampoos for some time. While men are advised to use 5%minodixil lotions, women are advised to use the 2 % version.

Pathophysiology

  • When the men and women reach their middle ages, it is common to see indications of male pattern baldness in both. This is the primary cause of hair loss. The reason lies in our genetic structure. When the male hormone testosterone gets converted to another derivative
  • 5-dihydroxytestosterone (5-DHT), the hair follicles in men start becoming finer as the age advances. This happens usually in genetically predisposed men. It usually starts as restricted hair growth and finally leads to complete disappearance of hair from the head.
  • As we all know, hair continues to change throughout its life. The hair undergoes three phases during its lifetime-
  •   The first stage is the anagen phase which marks the start of the active growing stage.
  •   Then the second stage is the catagen stage which is marked by morphological changes in the hair.
  •   The third stage is the telogen phase which is more of a resting period. During the anagen phase, the hair grows at a rate of 1-2 cm every month. This phase lasts for 2-4 years. Telogen phase usually lasts for around 100 days.
  • Usually, humans tend to shed around 40 -100 hair strands daily, which gets replaced by new hair. When the shedding exceeds the replacement rate, the hair loss becomes evident.
  • While males tend to show hair loss in the crown and the central area of the head, the females show increasing in the parting of the head and gradual thinning of hair.It is possible to have men hair transplant to solve the problem of male pattern balding.Similarlywomen hair transplant will solve the problem of hair loss in females.

Terminology

Terminal hair is the male type of hair which grows on the face and the body. It is androgen-dependent. Vellous hair is the hair which is fine and unpigmented in physical appearance. It usually covers the body in case of both the children and adults. When the FUG (follicular unit graft ) is done, the hair is taken from the terminal as well as vellous follicles. We can define a follicular unit as an entity which contains hair shafts from the scalp. The transplanted hair must follow the pattern and spacing between the natural hair so as to yield natural-looking results. So the harvesting process has to be carried out carefully. The pattern and spacing have to be kept in mind during the dissection and transplantation of hair as well.

Sequel

After two to three weeks of operation, the shaft of the transplanted hair, the epidermis and the dermis layer fall off as scabs. The follicles go into a resting phase after that. After about three months, new hair starts growing. With the use of minoxidil, the hair fall does not occur and the hair also starts growing almost immediately. The proper results of a hair transplant can be seen after 6 to 9 months of the procedure. If a second sitting is required, it should ideally be performed at least 3 to 6 months after the first sitting. Some patients might experience loss of sensation in the area for a period which could last up to 18 months in some cases. The density of the transplanted hair varies, it is thinner in areas which are totally devoid of hair. This information should be divulged to the patient before the operation. If needed, a second sitting may be done to increase the density.

Complications

  • Very few complications are seen in case of hair transplant.
  • It is rare to see infections in the recipient areas.
  • Infection may occur in the donor areas but it disappears after the removal of sutures.
  • In case epidermal cysts appear, they must be drained out.
  • The surgeon should be careful not to harvest a  large donor area as there should not be any tension on the suture line. Doing so will lead to a wide scar and high chances of the cuts splitting open.

Hair transplants in special sites

Eyebrows can be transplanted to give them a better appearance. While creating a new line, the surgeon must follow the direction of the eyebrow hair. Each eyebrow needs about 150 micrografts. The donor area for eyebrow must have fine hair, so it is usually taken from the nape of the neck.Hair from the temporal region can also be considered. After making the recipient holes with a gauge needle of 20 or 21 number, the procedure is initiated. A 0.7mm microblade can also be used for the purpose. The grafts are kept in place by using Cyanoacrylate glue.

Eyelashes may also be transplanted but it poses more challenges for the surgeon. It is a blessing that only a few lashes are required to make it successful. 5-6 one hair grafts are enough for the purpose. In this case, too ,Cyanoacrylate glue is used to keep the grafts in place.

It must be brought to the notice of the patient that the rate of hair growth is different in different regions. While the scalp shows a faster rate of hair growth, the eyebrows and eyelashes will show a slower rate of growth. The patients should also be told that the transplanted hair will need trimming on a regular basis.

Conclusion

Hair transplant has gained a lot of popularity in recent years. But one must be careful about the technical aspect as well. We must keep the following in mind-

  1. The surgery should be performed by a team as it is a difficult task to be carried out by a single person.
  2. All the team members must do their activities in an accurate manner.
  3. The surgery must be carried out in a well-lighted surgery room which must have all the instruments, of course of high quality.
  4. The surgery room must have a comfortable ambiance and the use of audio-visual entertainment can also be employed to make the patient comfortable.
  5. One must remember that it is better to stay away from the surgery if you are not sure of the clinic or the surgeon, as it might lead to worse looking results. You must ensure that you are opting for the best surgeon and the clinic to ensure that you get good results.

Expectations and Recovery

Post surgery, you might experience the tenderness of the scalp. You will be required to wear a bandage on the scalp for a period of 1-2 days. The surgeon will prescribe an anti-inflammatory drug for some days. With most of the people, if everything goes right, they are able to resume their normal work schedule within 2-5 days. The transplanted hair will fall out in 2-3 weeks and you will notice new growth after a few months. In most cases, around 60 % of growth can be seen after 6 to 9 months.

Risks

Transplants are considered quite safe but they too have their share of risks. Scarring and unnatural looking hair growth are to name a few. Some people may experience inflammation when new strands are growing. The inflammation of the follicles is called folliculitis. In such cases, antibiotics and compresses can provide relief. Some people also experience shock loss which, means a sudden loss of original hair in the area with the new strands. Usually, it lasts only for some time.

How Much The Treatment Costs

The question can’t be answered in one single line as the cost depends on a number of factors-

  1. Your location-the procedure cost will be according to the living standard of the particular area and the number of surgeons offering the treatment.
  2. The type of procedure –The two types of hair transplant procedures FUT and FUE have different costs.
  3. The expertise of the surgeon-While high rates don’t necessarily mean a good surgeon, the viceversa may be true. An expert surgeon will charge a higher fee no doubt.
  4. The amount of hair transplanted-If the procedure pertains to only a few patches, definitely it will cost lesser than hair transplant for the entire scalp.
  5. Travel cost-Though this will not be directly a part of your medical bill, it will still be borne by you. If the doctors nearby are not competent enough, you will have to travel to the desired location and pay for it.

Hair Restoration

Hair restoration has become a popular medical procedure with FUT and FUE methods. The FUE method used to be a labor-intensive method, which required a large number of people to participate in the process. One session could only guarantee a transfer of 300-60 follicles. The hair follicle survival rate was also less. With the automation of FUE, these limitations were eliminated. It is now offering-

  1. The possibility of  1500-2000 grafts per session.
  2. Guarantee of   less scarring
  3. Less downtime and quick recovery.
  4. It also guarantees fewer side effects and complications.
  5. Easier post-operative care
  6. Less pain
  7. The possibility of wearing a short hairstyle

Usually, hair transplant uses grafts from the donor area of the scalp, if there is sufficient growth at the back and near the ears. But if the scalp doesn’t have sufficient hair growth in these areas,it is suggested that he opts for body hair transplant. This means that the donor grafts are extracted from other areas of the body which have hair. The procedure begins after all the test reports come out to be normal and the test graft shows positive results. It has the following steps-

  1.    The donor area on the other part of the body (chest,  thighs, arms), etc is marked and anesthesia cream is applied. If that is not sufficient,  then tumescent anesthesia is used.
  2.    The grafts are extracted and kept in Ringer lactate solution.
  3.    The grafts are transferred to the various parts of the scalp which show poor hair growth.
  1. The donor area starts healing after 2 days.
  2. The recipient area shows initial hair growth after the fourth month and 3-4 cm of hair growth after 8 months
  3. Regular follow-ups must be taken

If you thought hair transplant is only limited to the scalp, you are wrong. Even facial hair like beard and mustache can benefit from the hair transplant technique. The methodology is as follows-

  1. The surgeon marks the area on the face which requires a transplant
  2. The transplanted hair follicles are extracted in such a manner that they match the facial hair. This is done using a custom designed implanting tool.
  3. The required amount and the extracted amount of hair follicles are counted.
  4. Implantation of the extracted hair follicles is done at the predetermined area.

The hair growth will continue to be there for the rest of the patient’s life after the beard transplant.

Mustache hair transplant for those patients who have a cleft lip or a scar in the area. We all know that hair in the mustache is wiry, thick and coarse. So it requires harvesting from the beard area. The area just below the jawline will be a good option for the donor area.Likewise ,beard hair transplant is also possible.

Patients who underwent a transplant in the older days have large plugs of hair which give the hairline a pluggy appearance. In the current approach, plug reduction and recycling are used which gives better-looking results.

Moustache hair transplant is an excellent solution for people who have a scanty mustache.

FUE technique is used to extract the grafts from the donor site, which is usually the back of the scalp. These grafts are implanted on the bald patches of the mustache. It usually takes only a single sitting to cover all the patches. The results are satisfying. The transplanted hair can be grown and trimmed like the normal hair

Alopecia areata is an autoimmune disease which shows up as hair loss in an unpredictable manner. The immune system of the body starts attacking the hair follicles, thinking that they are foreign bodies. This results in the complete destruction of hair follicles and leads to balding. The disease can be treated with hair transplant as there is no other procedure to stimulate hair growth in such cases. The surgery is performed after clearly marking the donor and the recipient area. It is usually recommended that the surgery is carried out before it is too late and all the follicles have been destroyed.

If the hair loss is excessive, then the restoration process might require many sessions. We understand the paucity of time the patients have. Keeping that in mind, the Giga sessions have been invented. Following are the main points related to a Giga session-

  1. It is a combination of FUE from the body and the scalp.
  2. It involves the extraction of hair follicles from the scalp, beard, chest, and legs so that maximum grafts are available.
  3. Around 5000 grafts are extracted at one time.

What are the prerequisites for a Giga session?

  1. Sufficient amount of grafts supply
  2. A capable team of the surgeon and the members, they must possess the capability to carry out the activity.
  3. A high capacity detector must be employed (greater than 3000 follicles per hour)
  4. The implanter speed should be also high. It should exceed 400 grafts per hour.

Hair Transplantation Procedure

If one were to summarize the procedure of hair transplant, it would look something like this –

Preparation For The Hair Transplant

This is the first step of the surgery. In it, the hair follicles are removed from the donor area(usually back of the head) and relocated to the balding (recipient )area.

Donor area trimmed

Prior to the surgery, the donor area hair is trimmed.

Donor area prepared for surgery

After trimming, the anesthesia is given to the donor area.

Tissue in the donor area removed and the donor area is sutured.

After this, the tissue of the donor area is removed through a surgical process and then sutured by the surgeon.

Combed hair over sutured donor area

The sutures in the donor area are covered by the patient’s hair. The removal of sutures is done after a period of ten days.

Donor tissue trimmed into follicular unit grafts

Dissection of the donor tissue is done by the surgical technicians using microscopes. After dissection, the follicular unit grafts are prepared.

Bald recipient area prepared

The recipient area is made ready for the process. The recipient area does not require the administration of anesthesia.

Incisions made in the balding areas

Follicular grafts are placed in the incisions made in the recipient area. The incisions are made in an irregular manner, therefore the grafting also takes place in an irregular manner.

Grafts placed according to their densities

It involves the placement of the grafts according to their densities. While the smaller ones are placed in the front, the denser ones are placed at the back.

Immediately after the Hair Transplant Surgery

Immediately post surgery, one can observe tiny incisions with minute hair on the operated area.

The closing of the Hair Transplant Surgery

The incisions marks heal which can be seen by the disappearance of the redness in the recipient area. This happens after a week’s time.

Recuperation Time

Hair transplant doesn’t take much time to heal as it is a non-invasive procedure. The patients can shampoo their hair after a period of 2-3 days. But the scalp must be protected from the sunlight and infections. The patient is generally advised to complete a course of antibiotics after the surgery.

Recuperation Time

The surgery is an easy one, but one must address the following issues before opting for it –

  • Have realistic expectations. If you didn’t have much hair in the first place, you can’t expect a miraculous growth on your head after the surgery. If you have a good density in the remaining hair, it will show in your treatment as well.
  • Think about the finances– You will have to shell out the money for the surgery as it will not be possible to get an insurance cover for it. It is better to discuss the cost break up with the surgeon in detail so that no confusion remains in your mind.
  •  Evaluate your risk factors– Smokers face a risk of complications from the surgery. Smokers are advised to quit smoking prior to the surgery.
  • A surgeon with proven skills-Select a surgeon with a good track record and excellence in the field. The skills of an experienced surgeon will definitely matter.

Complications Of Hair Transplant Surgery

As it is with all the surgeries, hair transplant also has its share of risks-

  • An allergic reaction to anesthesia. It may prove fatal in some cases.
  • Risk of bleeding and infection.
  • Permanent loss of sensation due to   nerve damage
  • Permanent damage of the skin grafts
  • Risk of tissue death
  •  Further surgery may be required to treat the complications
  • Some risks might be specific to your medical history or lifestyle. It is essential to speak to your surgeon about the individual problems you might face.

Why Is India The Best Place To Have A Hair Transplant?

India is the ideal destination for many people for getting their transplant done. It is advantageous to undergo hair transplant in India.
