    We have already mentioned that the surgeon needs to be careful while making the clots in the head. While scalpel blades are not the ideal instruments for making holes, other instruments are not easily available in India. The scalpel is unsuitable because it often cuts deeply resulting in floating of the inserted grafts. They also cause significant damage to the hair in case the patient is a woman. To eliminate such complications, an instrument called ‘Kolkata Slit ‘ has been recommended.  It is an instrument which can be reused and available in different sizes. The instrument is effective in ensuring that the gap created is just according to the size of the graft and the direction of orientation is maintained. The instrument is also effective in increasing the density of areas with scanty hair growth.

    Graft Insertion

    In this step, the grafts are placed into the recipient slits. The instrument used for this purpose is fine angled forceps. Minimum damage is desired, therefore the grafts are extracted in such a manner that about 2mm of subcutaneous fat is extracted. Using steady pressure the grafts are positioned on the recipient site. The surgeon must avoid burying the grafts below the skin level as it will lead to an unnatural appearance and lead to the formation of cysts. A team of two or three persons can be used to insert the grafts to endure efficiency and speed. A typical session lasts up to 5-6 hours which involves the grafting of 200-30 grafts.

    Postoperative care

    Usually,the patient can be discharged the same day. Most of the surgeons don’t prefer bandaging the area but some may do it. If the area is bandaged, then it must be done carefully. Removing the bandage too will require a lot of care as the grafts might stick to the surface of the bandage.

    Redness and swelling to some extent are expected and the patient should be prepared for it. Surgeons might use oral steroids for 3-5 days to reduce edema. Some medicines might be injected to reduce the swelling. The patients are usually advised to wear a headband on the face to avoid the swelling from coming down to the level of the face. The patient can wash his hair with a mild shampoo on the second or third day. The patient must be careful not to strike the tooth of the comb against the transplanted hair while combing.

    The patient must avoid wearing tight t-shirts which need to be taken off over the head. The patient is also advised to not to use hair oils and strong shampoos for some time. While men are advised to use 5%minodixil lotions,women are advised to use the 2 % version.

    Pathophysiology

    • When the men and women reach their middle ages, it is common to see indications of male pattern baldness in both. This is the primary cause of hair loss. The reason lies in our genetic structure. When the male hormone testosterone gets converted to another derivative
    • 5-dihydroxytestosterone (5-DHT), the hair follicles in men start becoming finer as the age advances. This happens usually in genetically predisposed men. It usually starts as restricted hair growth and finally leads to complete disappearance of hair from the head.
    • As we all know, hair continues to change throughout its life. The hair undergoes three phases during its lifetime-
    •   The first stage is the anagen phase which marks the start of the active growing stage.
    •   Then the second stage is the catagen stage which is marked by morphological changes in the hair.
    •   The third stage is the telogen phase which is more of a resting period. During the anagen phase, the hair grows at a rate of 1-2 cm every month. This phase lasts for 2-4 years. Telogen phase usually lasts for around 100 days.
    • Usually, humans tend to shed around 40 -100 hair strands daily, which gets replaced by new hair. When the shedding exceeds the replacement rate, the hair loss becomes evident.

    Terminology

    Terminal hair is the male type of hair which grows on the face and the body. It is androgen-dependent. Vellous hair is the hair which is fine and unpigmented in physical appearance.     Sequel

    After two to three weeks of operation, the shaft of the transplanted hair, the epidermis and the dermis layer fall off as scabs. The follicles go into a resting phase after that. The proper results of a hair transplant can be seen after 6 to 9 months of the procedure. If a second sitting is required, it should ideally be performed at least 3 to 6 months after the first sitting. The density of the transplanted hair varies, it is thinner in areas which are totally devoid of hair. This information should be divulged to the patient before the operation.     Complications

    • Very few complications are seen in case of hair transplant.
    • It is rare to see infections in the recipient areas.
    • Infection may occur in the donor areas but it disappears after the removal of sutures.
    • In case epidermal cysts appear, they must be drained out.
    • The surgeon should be careful not to harvest a  large donor area as there should not be any tension on the suture line. Doing so will lead to a wide scar and high chances of the cuts splitting open.
